| SUPPORTED RESIDENTIAL SERVICES (IN-HOME) |
| CFS, Inc. provides one-on-one, in-home support, assistance, training and specialized supervision services to children and adults with mental retardation or developmental disabilities who are approved for the DD Waivers. This program promotes a climate of dignity and respect with the goal of community integration, client choice and quality of life. CFS, Inc. empowers all individuals to hire their own service provider. All individuals are offered the necessary assistance to interview potential employees. |

| CRISIS STABILIZATION/ CRISIS SUPERVISION |
| CFS, Inc. provides crisis stabilization and supervision services for DD Waiver recipients who are experiencing serious psychiatric or behavioral problems that are jeopardizing their current living situation. This service is temporary and designed to prevent out of home placement. This service is only available to existing clients. |
